Minutes of Special Board Meeting
October 30, 2003

 

 

CALL TO ORDER

The Special Board Meeting of the Nevada State Board of Nursing was called to order by the President, Patty Shutt, LPN, at 10:00 a.m. on October 30, 2003, at the Grant Sawyer Building, Room 1100, 555 E. Washington, Las Vegas, Nevada 89101.

APPROVAL OF CONSENT AGENDA

 It was moved and seconded to approve the Consent Agenda, identified as items D.2.a, D.8.a,c.  MOTION CARRIED.

1.      Applications (initial and renewal) in question:

a.       Beckwith, Diane, RN12032: It was moved and seconded the Board accept the renewal application for Registered Nurse and issue a license with limitations pursuant to NRS 632.307.  It shall be the responsibility of the applicant to use her nursing judgment to practice safely in accordance with her limitations.  Unsafe practice is considered unprofessional conduct and is subject to Nevada Revised Statutes (NRS) Chapter 632.  MOTION CARRIED.

c.       Coleman, Kim, RN31500: Applicant was present. It was moved and seconded the Board accept the RN renewal application.  MOTION CARRIED

d.      Denkberg, Patricia, RN07076:  This item was removed from the agenda.

e.       Dennis, Laura, RN Applicant:  Applicant was present. It was moved and seconded the Board accept the application for licensure. MOTION CARRIED

i.         Jones, Douglas, RN applicant:  It was moved and seconded the Board accept the application for licensure. MOTION CARRIED.

j.        Makhlouf, Joanne, LPN04145: It was moved and seconded the Board accept the renewal application for Licensed Practical Nurse and issue a license with limitations pursuant to NRS 632.307. n.       Houlihan, Olive, RN3052: This item was removed from the agenda.

o.      Parrett, Frances, RN applicant: It was moved and seconded the Board accept the application for licensure.  MOTION CARRIED.

p.      Mc Avoy, James, RN applicant:  It was moved and seconded the Board accept the application for licensure. MOTION CARRIED.

q.      Hays, Phillip, RN20793:  It was moved and seconded the Board accept the RN renewal application and the Agreement for Probation (Non-Disciplinary) pursuant to NRS 632.320 (5) habitual intemperance.  MOTION CARRIED.

r.        Benson, Fatima, CNA applicant: Applicant was present.  It was moved and seconded the Board accept the application for certification. MOTION CARRIED.

s.       Baker, Brenda, RN applicant: Applicant was present.  It was moved and seconded the Board accept the application for licensure. MOTION CARRIED.

t.        Taylor, Tabbly, RN applicant: Applicant was present. It was moved and seconded the Board accept the application for licensure.  MOTION CARRIED.

u.       Price, Timothy, CNA applicant: Applicant was present. It was moved and seconded the Board accept the application for certification. MOTION CARRIED.

v.       Hursh, Melinda, RN applicant: Applicant was present.  It was moved and seconded the Board continue this matter to the November 2003 Board meeting. Applicant is to have an evaluation by the Disability Advisory Committee prior to the November Board meeting.  MOTION CARRIED.

w.     Andaya, Rommel, CNA applicant:  Applicant was present. It was moved and seconded the Board close the meeting to consider the character, alleged misconduct or professional competence of the applicant. MOTION CARRIED.  It was moved and seconded the Board re-open the meeting. MOTION CARRIED.  It was moved and seconded the Board accept the application for certification. MOTION CARRIED. 

x.       Pollack, Marta, RN23782:  Applicant was present. It was moved and seconded the Board accept the renewal application for licensure.  MOTION CARRIED.

y.       Smith, Taylor, RN applicant: Applicant was present.  It was moved and seconded the Board accept the application for licensure.  MOTION CARRIED.

 

2.      Settlement Agreements:

a.       Mosby, Robert, CNA005781: Voluntary Surrender of License/Certificate in Lieu of other Disciplinary Action by the Board for violations of NRS 632.320 (14) violation of a Board order. Accepted under the Consent Agenda.

 

3.      Completion of probation:  No business.

 

4.      Reinstatement of licensure/certification:

a.       Suarez, Gabamel, CNA003053: Applicant was present. It was moved and seconded the Board accept the reinstatement application for certification.  MOTION CARRIED.

 

5.      Formal administrative hearings:  No business.

6.      Board-directed investigations:

a.       Kentfield, Sarah, RN30455:  It was moved and seconded the Board direct staff to investigate.  MOTION CARRIED.

b.      Orvis School of Nursing:  It was moved and seconded the Board direct staff to investigate.  MOTION CARRIED.

 

7.      Ratification of staff denials – NRS 632.320 (1) fraudulent applications:

a.       Corpuz, Raquel, CNA Applicant:  Applicant was present to appeal the staff denial.  It was moved and seconded the Board ratify the staff denial of the application for licensure, based on a violation of NRS 632.320 (1) fraudulent application, and order the denial become part of the Respondent's permanent record and be published and reported to all appropriate agencies. In addition, should the applicant reapply for licensure within the next 30 days and if the application meets criteria for clearance by Board staff as stated in Board policy, it was moved and seconded that Board staff may clear the convictions and accept the reapplication.  MOTION CARRIED.

 

8.      Ratification of staff denials – NRS 632.320 (2) convictions:

a.       Konnor, Caleb, CNA applicant:  The Board ratified the staff denial of the application for certification based on a violation of NRS 632.320 (2) conviction and ordered the denial become part of the Respondent’s permanent record and be published and reported to all appropriate agencies.  Accepted under the Consent Agenda.

b.      Bojku, Imihane, RN applicant: Applicant was present with counsel to appeal the staff denial. It was moved and seconded to overturn the staff denial and accept the application for licensure.  MOTION CARRIED.

c.       Boley, Stephen, RN applicant: The Board ratified the staff denial of the application for certification based on a violation of NRS 632.320 (2) conviction and ordered the denial become part of the Respondent’s permanent record and be published and reported to all appropriate agencies.  Accepted under the Consent Agenda.

 

9.      Special Action

a.       Chawala, Kamala, RN, Appeal of licensure/certification denial:  It was moved and seconded the Board uphold the staff denial and deem the application acceptable for licensure by examination.  MOTON CARRIED.

b.      Whitfield, Susan, RN, Appeal of licensure/certification denial:  It was moved and seconded the Board uphold the staff denial.  MOTION CARRIED.  

 

E.     EDUCATION:  No business.

 

F.      NURSING PRACTICE:  No business.

 

G.    SPECIAL ACTION:  No business.

 

H.    DEBRIEF AND ADJOURNMENT
